1) Put the title of your story as the topic title. If it's a fanfiction then put the type in the topic title too. For example, if it's a HP fanfiction, then you would put that alongside the name as the topic title.
2) Rate your story using the link
here. This rating must be included in your topic title. You can expand upon the rating in your post.
3) You can give a summary of your story if you wish.
4) Make a thread in the reviews forum for your story. :)
